This is a fairly basic workflow form that is filled in by staff hiring new employees. It captures personal details, salary expectations, skills information etc. and then gets passed to more senior employees in the organization to either approve or reject the candidate choice.

Page 1 - Hiring Manager Captures Candidate Information
Having chosen the successful candidate for the job of Direct Marketing Manager, it is standard company procedure for placements as senior as this to be reviewed by other members of the company before the hiring takes place.
The Hiring Manager completes the first page of the form and is required to fill out most of the fields on this page. They will not be able to move to the next page until all the mandatory fields have been filled in. |

Page 2 - Further Candidate Information Required
Further questions about the candidate including compensation details, work location, working hours, IT requirements, etc. are asked. Having answered all the questions, the Hiring Manager presses the 'Next' button to show the final page of the form. |

| Page 3 - Hiring Manager Submits Form to Senior Staff Members for Approval
On this page the Hiring Manager checks the 'Approve' box, enters her name, email address and any comments. At this point all other fields on this page are read-only.
On pressing the 'Send' button, emails are immediately sent out to the Corporate Director, Corporate VP, CEO and Recruiting Manager alerting them to the form and the new proposed candidate.
A link in the email takes them directly to the form where they can review the candidate's details and either approve or reject the candidate or request additional information.
As soon as each person submits the form, the Hiring Manager is notified and may be required to obtain further information.
It is not until all staff have approved the proposed candidate that the Hiring Manager is able to make the job offer.
